What we collect
When you fill out a form on this site, you give us:
- Your name
- Your phone number
- Your email address
- What you're looking for — the option you select from the list
Your browser also provides, automatically:
- Which page you were on and which link or ad brought you here
- Approximate location based on your IP address — city level, not an address
- Basic device and browser information
What we do with it
We use it to contact you about your inquiry. Someone from the clinic will call you, usually within 15 minutes during business hours, and may follow up by text or email.
We don't sell your information. We don't rent it, trade it, or share it with anyone who wants to market something else to you.
Who else sees it
Only the services required to run this website and reach you:
- The clinic's patient management system, where the clinic's team works your inquiry
- The email service that notifies clinic staff that you reached out
- The hosting provider that serves these pages
Each of these acts on the clinic's instructions and for no other purpose.
Advertising platforms do not receive your personal information.
This site uses a Meta (Facebook) advertising tool. It records that someone submitted a form, so we know which ads are working — but your name, phone number, email, and what you selected are never sent to it. That information goes only to the clinic. We verified this in testing before launch.
Cookies
This site uses cookies to remember your session and to measure which ads bring people here. You can block or delete cookies in your browser settings. The forms will still work.
How long we keep it
Inquiries are kept in the clinic's patient management system for as long as needed to respond to you and to keep the records the clinic is required to keep. If you become a patient, your information is retained under the clinic's medical records policy.

- Ask us to stop contacting you — reply STOP to a text, unsubscribe from an email, or call us
- Ask us what we have about you
- Ask us to correct it or delete it

Children
